Dr. Sam is a dedicated chiropractor passionate about helping people move, feel, and live their best. Born in Madison and raised in Sheboygan, she brings genuine Midwest warmth and a patient-first approach to every visit.

She earned her Exercise Science degree from Marian University, where she competed on the NCAA women’s bowling team, giving her a strong understanding of movement, performance, and staying active throughout life. Dr. Sam went on to earn her Doctor of Chiropractic degree from Palmer College of Chiropractic in Davenport, Iowa. She began her career in Rockford, Illinois, working with professional athletes and serving as one of the team chiropractors for the Rockford IceHogs, the AHL affiliate of the Chicago Blackhawks. This experience strengthened her expertise in injury prevention, recovery, and performance care.

Today, Dr. Sam works with patients of all ages and activity levels, from children to athletes, with a simple goal: helping people move better, feel stronger, and get back to the activities they love.

Outside the clinic, Dr. Sam enjoys spending time outdoors, golfing, fishing, staying active with her dog Titan, and making memories with family and friends. She is also proud to serve her community as a firefighter with the Poynette-Dekorra Fire Department. Her passion for helping others extends beyond the clinic and reflects the same care, dedication, and sense of community she brings to her patients every day.

Meet Dr. Madison! She is the owner and head Chiropractor at Revive. When she is not in the office helping people feel the best they can, she can usually be found at Burn Bootcamp, listening to audiobooks, or spending some quality time with her family!
